Indiana POAs

This is the name we call the document required by the State of Indiana, in addition to a regular General Durable Power of Attorney, an in addition to a Designation of Health Care Representative, in case the Power of Attorney you have signed designates a different person to be your Healthcare representative, than you have designated to be your Durable POA lapse at death. This document permits specific additional powers to continue after death, including authority to make anatomical gifts, to request an autopsy, and to make burial arrangements.
