The HIPAA is a form that allows you to designate which persons, other than your assigned Healthcare Representatives, or your Attorney-in-fact are authorized to get information with regard to your health in the event that you are in the hospital and unable to give instructions at that time. Generally, this is a document that allows family members, extended family members, or family friends, to inquire as to your prognosis and to visit you in the hospital. With the stricter patient privacy laws this is the best way to assure your family and friends can learn of your health condition when you are hospitalized.

This is an advance directive that is usually provided by our office along with other advance directives, i.e., your Last Will and Testament, Power of Attorney, Indiana Healthcare Power of Attorney, Healthcare Designation, and Living Will.
